Dr. Perito and Dr. Suarez-Sarmiento Win VJSM Outstanding Video of the Year

At the 25th Annual World Meeting of Sexual Medicine in Rio de Janeiro on September 26, 2024, Dr. Paul Perito and Dr. Alfie Suarez-Sarmiento, Jr., were honored with the VJSM (Video Journal of Sexual Medicine) Outstanding Video of the Year award. Their award-winning video, titled “Double Distal Corporal Anchoring Stitch for Lateral Penile Implant Cylinder Extrusion,” was created in collaboration with Dr. Omer Raheem and Dr. Senthooran Kalidoss. This prestigious accolade was voted on by members of the ISSM (International Society of Sexual Medicine), recognizing the excellence and impact of their contribution to surgical technique in penile implant revision surgery.

The award was presented by Dr. Faysal Yafi, a well-respected figure in the field, who highlighted the significance of the technique outlined in the video. The team at Perito Urology was deeply honored to receive this recognition from their peers and extends their thanks to Dr. Yafi and the ISSM.

The video provides a detailed overview of the “Double Distal Corporal Anchoring Stitch,” an innovative method developed to address complications involving inflatable penile prostheses. Specifically, the ventral and distal aspects of the corpora cavernosa, known to be the thinnest areas, are prone to cylinder extrusion or crossover. The double distal corporal anchoring stitch technique offers a novel, corrective approach to secure and reposition extruded implant cylinders effectively.

Summary of the Technique: The video explains the surgical indications and the step-by-step process for the double distal corporal anchoring fixation stitch. Following a corporoplasty and capsulotomy, a lateral incision is made to locate the cylinder extrusion or crossover. The technique involves threading two Ethibond sutures through the distal cylinder eyelet and delivering each suture through the glans. By tying off the sutures, a bridge is created, securing the cylinder in place. Over the past four years, this technique has been used on 66 patients, achieving a satisfaction rate of 97%.

Outcomes have shown that this method significantly reduces the risk of further cylinder migration, with only two patients experiencing complications. The surgical success of this technique reinforces its clinical relevance, providing a reliable solution for this uncommon but challenging complication.
