Nearly 40 million men in the United States are affected by BPH, which occurs with advancing age when the prostate gland that surrounds the male urethra becomes enlarged and begins to obstruct the urinary system. Symptoms of BPH often include sleepless nights and urinary problems, and can cause loss of productivity, depression and decreased quality of life. Medication is often the first-line therapy for enlarged prostate, but relief can be inadequate and temporary. Side effects of medication treatment can include sexual dysfunction, dizziness and headaches, prompting many patients to quit using the drugs. For these patients, the alternative is surgery that cuts, heats or removes prostate tissue to open the blocked urethra.

During this meeting Dr. Perito was Faculty for two resident/fellow labs sponsored by Coloplast and held at the Boston BioSkills LAB and the Boston Convention Center. These labs combined comprised 8 rotations where residents and fellows got hands on experience practicing the “Minimally Invasive Penile Implant Technique”. These labs were followed up with Q&A sessions which allowed attendees additional time to ask specific questions about various aspects of the procedure.
During the meeting Dr. Perito spoke at the SMSNA (Sexual Medicine Society of North America’s) Committee meeting where he discussed “Pearls for Avoiding the Common Pitfalls of the Infrapubic Incision in Penile Prosthesis Surgery”.
Lastly, Dr. Perito co-chaired a course titled “Paradigm Changes in a 40-year old operation: Inflatable Penile Implant (IPP)” with Dr’s Steve Wilson and Robert Valenzuela.
Dr. Perito co-authored the following abstracts presented through either poster or podium presentation during this meeting.
1. It can happen to any man.
Although Peyronie’s disease most commonly affects middle-aged men, it can happen to men as young as 20 and well into the later years of their lives. Research suggests that between 8-10% of men will experience Peyronie’s in their lifetime. I am sure that this number could be higher if one accounts for untreated and undiagnosed cases.
2. It can impact or halt your sex life
While most cases of Peyronie’s disease have only mild symptoms, moderate to severe symptoms can cause painful erections, pain with intercourse, a crooked appearance in the penis, and potentially erectile dysfunction. As the symptoms often manifest slowly, as well as the potential embarrassment that can accompany sexual dysfunction, many men will delay seeking any kind of treatment until Peyronie’s has devastated their anatomy and sex life.
3. It’s treatable.
In milder cases, doctors will suggest simply waiting 1-2 years to see if symptoms subside, but there are a variety of prescriptions and surgical procedures that can be used to treat more serious cases. Xiaflex, an injectable drug, has been shown to cause light to moderate reductions in the plaque which can cause the penile bending effect seen in more severe Peyronie’s cases.
In moderate to severe cases, surgery is often the best course. Dr. Paul Perito has pioneered the ‘Scratch Technique,’ in which an incision is made into the penis to physically remove plaque-ridden areas, and is then supplemented with a penile implant to straight the penis and restore sexual function.

Dr. Paul Perito spoke about the top men’s sexual health issues in a panel discussion the South Florida Hispanic Chamber of Commerce’s Diversity Health Fair on April 27, 2017. Dr. Perito represented Coral Gables Hospital and was joined by physicians with a range of specialties on the panel. The discussion theme was “being healthy is not a goal; it’s a lifestyle.” Every year, the Diversity Health Fair attracts more than 1,000 visitors. Entertained visitors left the Fair pledging to have as much healthy sex as they can as they strive towards a healthy lifestyle.
Dr. Paul Perito and Perito Urology performed five advanced surgical repairs on penile implant patients experiencing prior complications during a training program titled “Difficult Case Day.”
While penile implant surgery has a low rate of infection and complication when performed by a well-trained surgeon, it is sometimes necessary to revise prior surgeries if that patient is experiencing adverse effects. On this day, the five patients treated by Perito Urology endured 25 previous implant-related procedures. Through a variety of surgical tricks all five patients received functioning inflatable or malleable penile implants.
Repairing complicated penile implants is delicate work, as the patient is often experiencing symptoms such as infection, pain, or device malfunction which can add an extra level of difficulty to what is already a challenging procedure. There are only a handful of urological surgeons who are able to achieve favorable outcomes from complications of this magnitude, which is why patients from all around the world travel to Perito Urology.
